Health Connections is a 24-hour health information and physician referral line provided as a community service by Stormont-Vail HealthCare. Staffed by 36 specially trained registered nurses and resource representatives, Health Connections will assist you with your health care questions and in finding a physician or community service that's right for you. Our resource representatives also register callers for educational classes and health screenings offered by Stormont-Vail HealthCare.

The Alzheimer’s Association provides a wealth of community programs and services through its national network of chapters. Typical programs and services offered by local chapters are described below. In addition, some chapters offer special programs such as assistance to persons with Alzheimer’s who live alone, rural and/or multicultural outreach, care coordination services, and training programs for families and professionals.

Typical programs and services

Helpline — Chapters offer a telephone service that provides emotional support to the caller as well as information about Alzheimer’s disease and related disorders, chapter services, and community resources. They also maintain updated information on community resources such as home care, adult day care, care coordination, assisted living, skilled nursing facilities, eldercare lawyers, financial planners, and transportation. Additionally, they can help individuals with Alzheimer’s disease and their families assess whether a specific care provider can meet their individual needs.

Support Groups — Peer- or professionally led groups for caregivers and others dealing with Alzheimer’s disease are another chapter-provided service. Specialized groups are available in many locations for children, individuals with early-onset or early-stage Alzheimer’s, adult caregivers, and other groups with specific needs.

Education — Chapters maintain a wealth of educational materials (brochures, videos, audiotapes, books, etc.) on topics related to Alzheimer’s disease and related disorders — basic information about these conditions, getting a diagnosis, communication techniques, home safety tips, choosing a residential care setting, experiences of other caregivers, activity programming, etc. They also offer a variety of ongoing educational programs targeted to specific audiences — the general public, individuals with the disease and their families, and health care and social services professionals — as well as customized speaking engagements upon request. Some educational programs offer continuing education (CE) or continuing medical education (CME) credits.

  • Support Groups: Referral to all cancer support groups for cancer patients and families.
  • Road To Recovery: Transportation for cancer patients to and from chemotherapy or radiation treatment.
  • Look Good… Feel Better: A free program to help cancer patients improve appearance and self image with hands-on beauty techniques demonstrated by a certified cosmetologist.
  • Guest Room: A cooperative program with participating hotels or other lodging facilities to provide housing for patients while receiving outpatient treatment.
  • Camp Hope: A free camp for children ages 8-21 who have, or have had cancer.
  • Man-to-Man : Education and support program for men diagnosed with prostate cancer.

Stroke Support System

Stroke Support Groups and Stroke Peer Visitation Programs: Research has demonstrated the importance of the social aspect of recovery from stroke. The American Heart Association maintains stroke support programs at the national and local levels (see American Stroke Association/Stroke Connection). Key among these are local stroke support groups and Peer Visitor Programs.

The Heart Walk is a non-competitive walking event that's geared to companies. Participate by forming teams of employees, family members, friends and others. Join thousands from coast to coast and participate.
