What is home health?
Home health is physician-directed, intermittent health care provided through scheduled visits. Care is provided by professional caregivers to homebound patients with skilled requirements when home is the appropriate and safe place to be. A registered nurse is on call 24 hours a day.
Eligibility
Who is eligible?
Adults living in Shawnee County who are currently under a physician’s care, who have a need for professional nursing care or rehab services, and are capable of living safely in their own home with referral to & support from community resources as necessary.
Delivery
How are services delivered?
Services are delivered in the patient’s home as directed by the patient’s physician. A plan of care is established in partnership with the patient, their physician, and family/caregivers to promote his or her maximum level of wellness and independence.

- Medical Social Work
- Home Health Aide
- Skilled Nursing, including:
- Free in-home assessment
- Wound care
- Drawing blood
- Medication monitoring and set-up
- Patient education
- Catheter care
- Chronic health condition monitoring
Availability
When are services available?
Office hours are Monday thru Friday, 8:00 a.m. - 5:00 p.m. Nurses are available on-call 24 hours a day, seven days a week.

What are the costs?
Fees for services are based on actual costs. Most Home Health Care costs are covered by Medicare, Medicaid, and some commercial health insurance carriers. Benefits and co-pays vary from plan to plan. Call your insurance carrier or Shawnee County Home Health for more specific coverage information.
