Health Department: Adult Field Services: Gatekeepers Assessment Team

A Gatekeepers Social Worker consults with a client in her home to create a plan of action that will provide better access to medical care and transportation to community services.

What is the Gatekeepers Assessment Team?

Gatekeepers is a Shawnee County Health Agency program that is meant to help any adult living in Shawnee County have the opportunity to be connected to health care, mental health services, appropriate housing, emergency aid services, and support services. A social worker/nurse team will help you plan ways to solve your problems. If you choose to use Gatekeepers services, you will be an active and involved member of a team that can help you achieve your goals for a healthier and more secure life.

Who is eligible?

Residents of Topeka or Shawnee County who are 18 years or older

Delivery

How are services delivered?

Services are delivered in the client’s home or another location chosen by the client. They are provided by a licensed social worker and a registered nurse.

  • Nursing Assessment
  • Social Assessment
  • Referrals for Medical Care
  • Assistance with connections to mental health services
  • Referrals to community resources
  • Follow-up support services

 

Note: Gatekeepers cannot provide ongoing skilled nursing care.

To arrange services from Gatekeepers, you may call the KCSL Family Resource Center Community Intake hotline at 354-8902.

Office hours are Monday thru Friday, 8:00 a.m. - 5:00 p.m.

Cost

What are the costs?

All services delivered by the Gatekeepers Assessment Team are provided without cost to the consumer. Certain community-provided services, however, may require a fee payable to the service provider.

Volunteering

Can I volunteer?

Gatekeepers are community members and neighbors who notice when others may not be able to do things the way they always have.  They may see changes in behavior or ability.  They may worry that something just doesn’t seem right.  If you would like to be trained as a Gatekeeper, please call 785.291.2488 to arrange for you or your group to receive training. It only takes about an hour.
